Step 1 was changed to pass/fail in 2022 to reduce stress and burnout. But medicine didn’t suddenly become less competitive. The pressure didn’t disappear. It shifted.
What actually changed after Step 1 became pass/fail, who felt it most, and what does it mean for staying competitive for residency?
They should just have one exam for each year in addition to the step 1 and 2. Stress is part of life. I get stressed and? We have to learn to manage it.

I recall that students who are disadvantaged typically do better on Step 2CK than Step 1 because they gain more exposure to medicine and science through rotations. If that's still true, then it's a benefit to having more weight on Step 2CK.

Can't we argue that if we do NEED a bottleneck exam, it might as well be Step 2 rather than Step 1. It's far more useful and clinically relevant compared to the random fact check that step 1 requires.
But even this argument is shortsighted. People need to be asking the real question of whether the USMLE actually captures the qualities of a competent and good physician. How about the other metrics med students are graded on ( class rank, research items, subjective evaluations) do these actually create a better, more knowledgeable, and compassionate workforce of doctors?

My biggest problem with Step is 1/4 of the questions not counting towards your score as "experimental" i.e. doing free work for the NBME and paying a lot of money for the privilege.
How is it there are fewer question on the Internal Medicine Board Exam than Step 1?

It was a huge mistake. My class students were focused more on research the first and second year then they should have been. They should have been learning the fundamentals. And third year they are now rushing home to study for step two rather than taking in the experience of their clinical rotations.

The USMLE (NBME) question writers can only reword old questions. The questions are terrible. I am using UWorld right now and it includes questions that most 4th year students cannot answer. It has become a memorization exam. Students just run through endless questions everyday (100-150) and memorize Anki cards. Actual learning is significantly diminished and rote memorization is heavily rewarded.

I don't believe in any of those numbers talking about research... 34.7 research items in 4-5 years? That's like 9 research projects per year. I need a month to just write a proper paper! Everybody is cheating adding their names to research projects. People literally know other people and do something very minimal or do nothing at all and they put their names in the paper.

In Canada there is no step 1. There is an exam LMCC (MCCQUE1) that you write after matching which is clinical in nature, and which almost everyone passes.
The US is stupidly obsessed with standardized testing. Step 1 is useless all it tests is basic science. Clinical testing and experience are the only things that are important in medicine.

@thunderfreeze1544USMLE Step 1 is extremely necessary. It has now been replaced by rushing to publish papers, most of which have little value and are generated using AI. In the future, it will be proven that this kind of reform is a mistake.Even though it is now only reported as pass/fail, I will still insist on reaching a top 1% level within my first year of the MD program. I firmly believe this will be extremely meaningful for my future growth.For research, I will pursue quality rather than quantity.
